PLEASE TAKE NOTICE that the undersigned Record Custodian/Declarant hereby places the above-named Recipient upon formal written notice of the following identified faults, defects, omissions, failures, breaches, injuries, and unresolved matters appearing within the Record of this Matter.
This Notice is issued in good faith and for the express purpose of providing Recipient a fair and documented opportunity to cure, correct, answer, rebut, perform, or otherwise resolve the matters herein identified prior to further procedural escalation.

Critical drafting rule:
do not be vague.
Each fault must be:
dateable,
describable,
document-supported,
individually answerable.
The recipient must feel they were given a real chance.
That makes later default far stronger.
